Waiver

I give my child permission to participate in the Faith Motivated Paintball Event on May 20th, 2017.I understand that there is possibility for injury while participating in the event, and agree to let the employees of the Faith Motivated Foundation act in their best judgment in case of sickness or injury. My signature below indicates that (1) I will not hold liable the faithmotivated foundation or pittsburgh paintball park if injury or sickness does occur, (2) my son/daughter is mentally & physically capable of participating in this camp, (3) my son/daughter is covered by medical insurance, (4) his/her participation is voluntary, (5) I voluntarily permit his/her participation and (6) I permit the use of his/her image for photos for marketing purposes.
